GUIDELINES TO PREPARE YOUR GEORGIA DOCUMENTS FOR THE APOSTILLE
BIRTH, MARRIAGE AND DEATH CERTIFICATES ACCEPTABLE FOR APOSTILLE
Georgia Birth, Marriage and Death Certificates should be obtained through the State Department of Vital Records (Contact Charity Warren, Costumer Relations). You should request for a Certified Copy with an original hand written signature (pen-in-hand) on each document. Some County Probate Courts issue Birth Certificates, which require a pen-in-hand signature. All County Probate Courts issue Marriage Certificates, which require a pen-in-hand signature. (Some County Probate Courts issue Birth Certificates, which require a pen-in-hand signature. All County Probate Courts issue Marriage Certificates, which require a pen-in-hand signature). To obtain a Birth, Marriage or Death Certificate contact Charity Warren, Costumer Relations Representative with the State Department of Vital Records Services at (404)-679-4701 or 4755. You must obtain a certified copy with a original and hand written signature (pen-in-hand) on each document.

BIRTH, MARRIAGE AND DEATH CERTIFICATES NOT ACCEPTABLE FOR APOSTILLE
No County Public Health Department documents will be acceptable for Apostille or Certification.

NOTARIZED DOCUMENTS ACCEPTABLE FOR APOSTILLE
The Georgia Notary Public who’s executing the document must put the proper text and exact sate of the notarial act as set forth in Georgia Notary Public Laws {O.C.G.A. 45-17-8.1(a)}. Information on the attached form can be hand written or typed into your documents, or it can be a separate page.

SECRETARY OF STATE CORPORATE DOCUMENTS ACCEPTABLE FOR APOSTILLE
The document must be a certified copy from the Georgia Secretary of State Corporations Division, and you must specifically request the original auto pen signature.
