        <description><![CDATA[This form is required by some states when a document is certified by a federal court for filing with a state or local authority. It is only necessary for the clerk to execute the form when a person requests that the form be completed.
For authentication of documents to be filed in overseas an apostille must be employed.  Only a district court clerk can issue an apostille.

        <description><![CDATA[In accordance with the Hague Convention of October 5, 1961, in which both the Russian Federation and the United States of America are participants, documents authenticated by an Apostille of a participant country are exempted from validation by the consular establishments of other participant countries. Documents with Apostille do not require any further legalization.
However, due to the different forms of execution of Apostille in Russia (seal) and in the United States (certificate), receiving organizations in Russia may ask for additional proof of proper legalization by consular offices of Russia in the US.

This simplified form contains standardized numbered fields of certain common and essential types of information, which allows the data to be understood by all participating countries regardless of the official language of the issuing country.

The completed apostille form certifies the authenticity of the signature on the documents, the capacity in which the person signing the documents has acted, and identifies the seal and/or stamp that the document bears.

        <description><![CDATA[Long forms of birth certificate, or certified photocopies (or book copies), are exact photocopies of the original birth record that was prepared by the hospital or attending physician at the time of the child\'s birth. The long form usually includes parents\' information (address of residence, race, birth place, date of birth, etc.), additional information on the child\'s birthplace, and information on the doctors that assisted in the birth of the child. The long form also usually includes the signature of the doctor involved and at least one of the parents.

Please note that to get an Apostille for a birth certificate or a death record was issued in one of the 5 boroughs of New York City (Bronx, Kings, Manhattan, Queens, Staten Islands), you will need to contact the Bureau of Vital records in NYC at 212-788-4520 and ask for the long form of the birth certificate or death record with a letter of exemplification.
