To get an Apostille for a birth certificate or a death record was issued in one of the 5 boroughs of New York City (Bronx, Kings, Manhattan, Queens, Staten Islands), there is a several process:

The size and color of all documents issued within the past 10 years are basically the same, even those which in fact are not acceptable. BIRTH, MARRIAGE AND DEATH CERTIFICATES ACCEPTABLE FOR APOSTILLE
Georgia Birth, Marriage and Death Certificates should be obtained through the State Department of Vital Records (Contact Charity Warren, Costumer Relations). You should request for a Certified Copy with an original hand written signature (pen-in-hand) on each document. Some County Probate Courts issue Birth Certificates, which require a pen-in-hand signature. All County Probate Courts issue Marriage Certificates, which require a pen-in-hand signature. (Some County Probate Courts issue Birth Certificates, which require a pen-in-hand signature. All County Probate Courts issue Marriage Certificates, which require a pen-in-hand signature). To obtain a Birth, Marriage or Death Certificate contact Charity Warren, Costumer Relations Representative with the State Department of Vital Records Services at (404)-679-4701 or 4755. You must obtain a certified copy with a original and hand written signature (pen-in-hand) on each document.

No County Public Health Department documents will be acceptable for Apostille or Certification.

NOTARIZED DOCUMENTS ACCEPTABLE FOR APOSTILLE
